If you are already a portal member, sign in here

 

If your information doesn't appear in the Follow My Health portal, make sure you have connected to Prime Health Network. 

To connect to Prime Health Network:

- Log into Follow My Health
- Click My Account
- Click My Connections
- Click Add Organization
- Search for Prime Health Network (All of our offices are grouped under our location at 301 West Chester Pike, Havertown PA 19083)
- Click Connect
- Wait for your connection approved email (allow up to 48 hours)
