Patient Portal Support

If your information doesn't appear in the Follow My Health portal, make sure you have connected to Prime Health Network. 

To connect to Prime Health Network:

- Log into Follow My Health
- Click My Account
- Click My Connections
- Click Add Organization
- Search for Prime Health Network (All of our offices are grouped under our administrative office address at 450 Park Way in Broomall)
- Click Connect
- Wait for your connection approved email (allow up to 48 hours)
