Check-in Policy

**Masks are still required**

Please arrive 15 minutes prior to your appointment.

We know that your time is valuable and try our best to stay on schedule, and appreciate your assistance in helping us to do so. If you are late for your appointment, we will do our best to accommodate you for that day, however, you may need to re-schedule for a later date.

It’s very important to make sure that all of your demographic and insurance information is current at EVERY appointment. Please be prepared to verify: 

  • Your address
  • Best phone number to reach you – we now offer text appointment reminders, please let us know if this phone number is a cell phone
  • Current e-mail address – Stay informed! We now send general practice information and educational materials through e-mail

In addition, we are requiring all patients to present the following at EVERY appointment:

  • Photo ID
  • Current Insurance Card – this is required to verify insurance coverage
  • Any referral forms  
  • Co-payments are due at the time of your office visit. For your convenience, we accept cash, check, and most credit cards.

Also, please remember to bring a list of any current medications (prescriptions and over-the-counter) that you are taking to review with your physician or provider.


We are committed to providing all of our patients with exceptional care. When a patient cancels without giving enough notice, they prevent another patient from being seen. Should you need to cancel or reschedule an appointment, please contact our office as soon as possible. As noted on our Cancellation Policy: We require that you contact the office no later than 3pm on the day prior to your scheduled appointment to notify us of any changes or cancellations. To cancel a Monday appointment, please call our office by 3pm the Friday before. This allows us time to schedule other patients who are on a waiting list. We realize that emergencies arise from time to time, however, please try to give us as much notice as possible when these situations arise.
